Job Description
As a compassionate Social Worker (MSW), you will be responsible for providing high-quality care to older adults in their homes. Your role will involve conducting comprehensive assessments, developing and integrating social services, and providing information and counseling to clients and families.
Responsibilities:
- Conduct comprehensive assessments to evaluate clients' health conditions and environment.
- Develop and integrate social services into the total plan of care, considering client/family perspectives.
- Provide information and counseling to clients, families, and caregivers for practical needs and long-term planning.
- Offer information and referrals to community services, acting as a liaison between clients and agencies.
- Deliver goal-oriented, short-term psychotherapy for social and emotional issues.
- Collaborate with team members to support client care and identify additional home care needs.
- Participate in discharge planning and address psychosocial functioning needs.
- Maintain clinical documentation for all referred clients.
- Foster relationships with public and private agencies as resources.
- Attend in-services, case conferences, and meetings to ensure coordinated care plans.
Salary : $28 - $35
