Social Worker (MSW) | Home Visits | Part Time
We are excited to welcome a compassionate Social Worker (MSW) to our Lakeshore Team, serving the vibrant communities of Allegan and Ottawa counties.
Responsibilities:
- Conduct comprehensive assessments to evaluate clients' health conditions and environment.
- Develop and integrate social services into the total plan of care, considering client/family perspectives.
- Provide information and counseling to clients, families, and caregivers for practical needs and long-term planning.
- Offer information and referrals to community services, acting as a liaison between clients and agencies.
- Deliver goal-oriented, short-term psychotherapy for social and emotional issues.
- Collaborate with team members to support client care and identify additional home care needs.
- Participate in discharge planning and address psychosocial functioning needs.
- Maintain clinical documentation for all referred clients.
- Foster relationships with public and private agencies as resources.
- Attend in-services, case conferences, and meetings to ensure coordinated care plans.
- Perform other position-related responsibilities as assigned.
Essential Skills and Abilities:
- Excellent verbal and written communication skills.
- Effective time and schedule management skills to consistently meet deadlines for charting.
- Proficiency with technology: tablets, mobile phones and EMR software.
Location and Schedule:
Allegan and Ottawa County. Day shift with flexibility in scheduling to help you maintain a healthy work/life balance. About 24hrs per week.
Qualifications:
- Master's degree in Social Work or related field / MSW certification.
- CPR certification.
- Valid Michigan driver's license, proof of auto insurance, and reliable transportation for home visits.
- 1-2 years post graduate social work experience in a health care setting.
- Passion for delivering high quality care to older adults.

Salary : $28 - $35
