Position Overview :
The Oncology Nurse Navigators will provide triage, support, and education to members during their cancer journey via phone, email, and video.
Responsibilities of the Oncology Nurse Navigators :
- The Oncology Nurse Navigators will establish trusting relationships with members and their care network.
- Support members throughout their cancer care journey, including screening, survivorship, and end-of-life care.
- Assist members with care coordination, symptom management, nutritional support, discharge planning, and provider referrals.
- Assist with urgent clinical escalations and provide clinical consultative support.
