Clinic LPN

Job Summary / Purpose:

The Licensed Practical Nurse (LPN) is responsible for performing and documenting assessments, ensuring completion of health history and admission processes, and facilitating interventions or referrals. The LPN identifies patient discharge planning and education needs, provides patient care, performs personal care tasks, communicates patient progress, and adjusts workload as needed.

Essential Key Job Responsibilities:

• Assessment and Planning:
• Assist the Registered Nurse (RN) or licensed practitioner in identifying patient needs, care priorities, and goals.
• Contribute to the evaluation and modification of the care plan through observation and communication of patient responses.
• Care Delivery:
• Execute department-specific tasks according to standards of care, policies, and procedures.
• Administer timely and safe nursing interventions and prescribed medical regimens while documenting accurately.
• Identify early warning signs of patient condition changes and report appropriately.
• Team Collaboration:
• Actively participate in a multidisciplinary team to implement integrated patient-centered care plans.
• Delegate or assign nursing interventions to qualified individuals, ensuring safety and competency.
• Share relevant patient information promptly with team members to ensure care quality and continuity.
• Education and Discharge Planning:
• Participate in health education as directed by a licensed practitioner.
• Assist in discharge planning by providing patient education and counseling to optimize understanding and compliance.
• Patient Comfort and Advocacy:
• Address patient-reported symptoms, including pain, promptly and according to established procedures.
• Provide personal care and emotional support to enhance patient well-being.
• General Duties:
• Perform other responsibilities as assigned to meet the department's needs.

Qualifications:

Education & Licensure:

• Current licensure as a Licensed Practical Nurse in the applicable state.
• Completion of an accredited LPN program.

Skills/Abilities:

• Effective communication and collaboration skills.
• Proficiency in documenting care accurately and timely.
• Ability to observe, recognize, and report patient condition changes.

Physical/Mental Requirements:

• Ability to perform physical tasks related to patient care, including lifting, positioning, and assisting with mobility.
• Strong decision-making skills and ability to manage stress in a dynamic healthcare environment.

Job Types: Contract, Travel nursing

Pay: $45.00 - $50.00 per hour

Expected hours: 36 per week
