Day Habilitation Specialist (BSW Degree Required)

Overview:

We are looking for a compassionate and dedicated Day Habilitation Specialist to join our team and work with individuals with developmental disabilities (DDD) in a day program setting. This role is perfect for someone with a Bachelor’s degree in Social Work (BSW) who is passionate about supporting individuals with disabilities in achieving personal goals, fostering independence, and enhancing their quality of life. The Day Habilitation Specialist will offer direct support, facilitate skill-building activities, and contribute to an individualized, person-centered approach to care.

Key Responsibilities:

• This is a hands-on role, with the majority of time spent working directly with program participants in the day program.
• Provide direct support to clients, ensuring their engagement in activities, assisting with daily living skills, promoting socialization, and encouraging community integration.
• Support clients in achieving goals related to personal independence, communication, life skills, and social integration.
• Facilitate group activities and structured programming, including arts and crafts, cooking classes, educational activities, and outings.
• Observe and track client progress, documenting changes in behavior, emotional well-being, and skill development.
• Report any concerns or changes in a client's well-being, including abuse, immediately to the day program director, assistant director, or director of nursing for investigation.
• Assist with intakes and admissions as assigned by the day program director.
• Provide case management for program participants as assigned by the day program director.
• Maintain professional relationships with clients, families, and other team members to ensure effective communication and collaboration.
• Work with the team (nurses, support coordinators, behavior specialists, direct support professionals, and family members) to implement individualized service plans for each client.
• Maintain accurate records and documentation on client participation, goals, and outcomes in accordance with DDD program guidelines and legal requirements.
• Attend mandatory weekly staff meetings and other required meetings.
• Supervise and monitor the whereabouts of program participants at all times.
• Provide crisis intervention and assistance during emergencies to ensure client safety.
• Follow infection prevention protocols.
• Foster a respectful, inclusive, and supportive environment that promotes the dignity and rights of individuals with developmental disabilities.
• Assist with transportation and accompany clients on outings or appointments as necessary.
• Participate in ongoing training and professional development to stay updated on best practices in the field of developmental disabilities and social work.
• Take on special projects and other duties as assigned by the day program director.
• Report directly to the Beyond 21 day program director.

Working Hours:

• This is a 40-hour workweek, with hours from 8:30 a.m. to 5:00 p.m., including a half-hour unpaid lunch.

Qualifications:

• A Bachelor’s Degree in Social Work (BSW) is required.
• Prior experience working with individuals with developmental disabilities in a day habilitation setting or similar social service role is preferred.
• Knowledge of developmental disabilities, person-centered planning, and behavioral management techniques.
• Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and multidisciplinary teams.
• Ability to maintain professional boundaries and confidentiality.
• Valid driver’s license and reliable transportation (if required).
• CPR and First Aid certification (or willingness to obtain upon hire).
• Strong problem-solving skills with the ability to respond to behavioral challenges calmly and effectively.
• A compassionate, patient, and respectful approach to working with vulnerable populations.

Working Conditions:

• This role may require occasional evening, weekend, or holiday hours based on client needs.
• The position involves physical activity, including assisting with mobility, supporting client participation in activities, and occasionally lifting or assisting clients.
• The Day Habilitation Specialist will work in various settings, including the program facility and during community outings.
