OverviewThe Manager of Extended Case Management plays a critical role in providing comprehensive support and guidance to clients facing various challenges. This position requires a compassionate and skilled professional who can effectively manage crisis situations, facilitate access to resources, and empower clients through motivational interviewing techniques.The Senior Case Manager will be managing a new department and will work collaboratively with community partners and stakeholders to promote workforce development and ensure clients receive the necessary services for their well-being.ResponsibilitiesLead and manage the Extended Case Management programs, including daily operations, staffing, and case activities.Develop personalized, holistic service plans and monitor client progress.Establish policies for client-centered case management, including benefits enrollment, healthcare access, housing, and referrals.Ensure staff receive training in trauma-informed care and maintain accurate records in EHR systems.Ensure program compliance with ORR, CalAIM, HIPPA, and other regulations.Oversee case note accuracy, compliance, and outcome tracking in various systems.Submit timely reports as required by funders.Collaborate with internal teams for referrals and partnerships.Build and maintain relationships with local service providers, community organizations, and healthcare professionals.Support short- and long-term strategy development and quality improvement for new or enhanced benefits.Ensure compliance with state and federal standards.Oversee interns, volunteers, and support staff, and stay informed of policy changes affecting programs.Supervise and support staff, including onboarding, training, and professional development.Provide clinical supervision for staff and interns.Conduct one-on-one meetings with direct reports for goal setting and performance review.Oversee program budgets, collaborating with relevant directors, and manage staff PTO, timesheets, and overtime.Perform additional tasks as assigned.RequirementsMUST have a Masters in Social Work (MSW)Associate Clinical Social Worker (ASW) working toward licensure -OR- 2 years active unrestricted license in California as a Licensed Clinical Social Worker (LCSW) required with experience providing clinical supervision2 years program administration experienceMinimum of 4 years in Case Management, health education, or related fields
