We are seeking a compassionate Registered Nurse to join our dynamic interdisciplinary team in providing comfort and support to patients and their families facing the end of life.
Job Summary
The RN Case Manager is responsible for coordinating care with an interdisciplinary team, ensuring patients receive appropriate levels of care based on their needs and preferences.
Key Responsibilities :
- Deliver nursing care within established standards, hospice guidelines, policies, and procedures.
- Assess and educate patients and families on home safety and navigating end-of-life care.
- Communicate and coordinate care with the interdisciplinary team as patient / family condition warrants.
- Create patient-specific plans of care / treatment in coordination with the interdisciplinary team.
- Document nursing interventions according to the nursing plan of care.
- Complete recertifications at appropriate time intervals.
- Function as a patient advocate.
