Auburn Crest Hospice is looking for a Registered Nurse who is excited to positively impact the lives of others during one of the most delicate times. The RN Case Manager provides compassionate care with clinical excellence. As an RN Case Manager for Auburn Crest Hospice you will join a team of interdisciplinary professionals to care for patients facing the end of life.
As the RN Case Manager you will be responsible for the overall coordination of care by the interdisciplinary team for the patients and their families. You will see that the patient is provided the levels of care appropriate to the patient and family needs. This position reports to the Director of Nursing.
The RN Case Manager for Auburn Crest Hospice is responsible for :
Delivering nursing care within established standards of care, hospice guidelines, policies and procedures.
Assessing and educating the patient and families on home safety and navigating end of life cares
Communicating and coordinating care with all members of the interdisciplinary team as patient / family condition warrants.
Completing initial and comprehensive assessments
Creating a patient specific plan of care / plan of treatment in coordination with the interdisciplinary team
Completing supervisory visits according to guidelines
Documenting nursing interventions according to nursing plan of care
Completing re-certifications at appropriate time intervals
Completing written verbal orders for any changes in medications or treatments or changes in level of care.
Functioning as a patient advocate
Certifications, Licenses and Registrations
Registered Nurse licensed in the state of employment.
Minimum of one (1) year nursing experience, preferably in a Home Health / Hospice setting within the past five (5) years is preferred.
Certification in Hospice and Palliative Nursing a plus but not required.
Current CPR certification.
Must have a valid driver’s license and auto liability insurance.

Strong clinical, organizational and interpersonal skills.
Strong written and oral communication skills.
Work Environment and Physical Demands
Duties are performed in an office and clinical environment and require sufficient personal mobility and physical capability to permit employee to function in this environment.
Must be able to lift up to twenty (20) pounds.
Occasional travel required.
