Job Summary
We are seeking a dedicated and compassionate Case Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the appropriate services and support necessary for their health and well-being. This position requires a strong understanding of various healthcare settings, including outpatient facilities, hospitals, and acute care environments. The Case Manager will work collaboratively with healthcare providers, patients, and families to develop comprehensive care plans that address medical, emotional, and social needs.
Duties
- Assess patient needs and develop individualized care plans based on clinical evaluations.
- Coordinate with healthcare teams in outpatient, hospital, ICU, and PICU settings to facilitate seamless patient transitions.
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Conduct utilization management reviews to ensure appropriate resource allocation and adherence to best practices.
- Provide education and support to patients regarding their health conditions and available resources.
- Collaborate with managed care organizations to navigate insurance processes and benefits.
- Maintain accurate documentation of patient interactions, care plans, and progress notes in compliance with regulatory standards.
- Advocate for patients’ needs within the healthcare system while fostering effective communication among all stakeholders.
Skills
- Strong knowledge of outpatient care processes, hospital protocols, ICU standards, and acute care practices.
- Experience in utilization management within managed care environments is highly desirable.
- Background in nursing or a related field is preferred; familiarity with infectious disease care is a plus.
- Solid understanding of human physiology to effectively assess patient needs.
- Excellent communication skills for interacting with patients, families, and multidisciplinary teams.
- Strong organizational skills with the ability to manage multiple cases simultaneously while maintaining attention to detail.
- Ability to work independently as well as collaboratively within a team-oriented environment.
