What are the responsibilities and job description for the Resident Care Coordinator/LPN position at Augusta Gardens Senior Living & Memory Care?
Trains Direct Care Staff on their scope of duties
Completes performance reviews for departmental staff members
Maintains adequate stock of supplies to complete departmental work responsibilities timely.
Is present in the Community during times of emergency and follows Emergency Preparedness plan including scheduling of staff members
Schedules Direct Care Staff members to meet Resident needs in compliance with staffing budget and state requirements including finding replacements for unplanned absences of scheduled staff.
Communicates effectively with Administrator keeping him/her well informed of resident and staff status on an ongoing basis, and by providing a daily written or verbal summary report of the events, actions taken, and action plans going forward.
Communicates with physicians, health agencies, and Residents’ Responsible Parties regarding resident care, services and resident health/condition status.
In cooperation with the Community Liaison, prepares all Resident records and documents for use upon admission and keeps them current.
Review and revision of Care Plan for significant Resident change in condition
Informs the Administrator, in writing, using the Concerns/Grievance form of any complaints and the interventions employed.
Monitors Direct Care Staff performance, providing or arranging for assistance and/or additional training as needed.
Monitors MAR for documentation compliance and addresses deficiencies appropriately informing Administrator, in writing, on a Medication Exception Report Form, of any non-compliance and corrective actions taken.
