What are the responsibilities and job description for the NAVIGATOR-General and Invasive Cardiology position at Augusta Health Brand?
The Cardiology Navigator helps the cath lab/cardiology patient navigate through the healthcare process throughout the duration of the care, beginning with first contact with the AMG Cardiology department continued on as long as they are under care of an AMG cardiologist. They are responsible for assisting with coordination of pre and post procedure care, including coordinating the scheduling of cath lab procedures as well as any supplemental procedures required. They will also be responsible for assisting with gathering all required patient documentation/data, i.e. – Lab work, EKGs, Holter monitors, echoes, etc... The Cardiology Navigator will round on patients post procedure, participating actively in care planning process and assessing for quality compliance. They will take an active role in the patient discharge process, participating in such activities as: discharge teaching/education, assisting with getting prescriptions filled, schedule post-procedure clinic visits, etc... The Cardiology Navigator will also collaborate with the Cardiovascular Quality Coordinator on process improvement around the NCDR measures and accreditation
Patient Navigation
• Coordinates the scheduling of procedures, diagnostic exams with schedulers
• Performing PAT visits/ phone calls.
• Ensures procedural patients are prepared (NPO status, appropriate lab work and medications adjusted appropriately)
• Gather/organize patient data prior to physician evaluation
• Patient/Family education: pre/post procedure
• Assist with inpatient and post procedure patient discharge planning
• Assist with post discharge medication coordination and meds to bed when applicable.
• Consults with and utilizes community resources to optimize treatment and support
• Responds to patient phone calls as needed.
• Educate patients and their significant others regarding their cardiac diagnosis and treatment plan
• Works with vendors and subject matter experts to organize and schedule cardiac education with clinic team members.
Report and Record Keeping
• Assists in tracking patient interventions and outcomes
• Participates in research as appropriate
• Tracks patients as they move through the healthcare system
• Provides timely and complete documentation for accurate patient care
• Tracks interventions and outcomes
• Helps coordinate and assists with ACC-NCDR data registry compliance
• Participates in the development, implementation, and evaluation of policies, procedures, quality assurance programs, and protocols.
Customer Service/Marketing
• Proactively works to resolve issues to improve quality and efficiency of department(s)
• Consistently displays professionalism in dealing with patients and peers

Requirements:
• BS in Nursing
• BLS required, ACLS within 6 months of employment
• Current RN licensure in the Commonwealth of Virginia
• Minimum of 2 years Critical Care, Cardiology, or Cath lab experience
Preferred Qualification:
- MS in Nursing or equivalent master's degree
