Responsibilities
The Registered Nurse Hospice Night Call provides skilled nursing care for Hospice home care patients following physicians’ orders and the established Plan of Care. The primary responsibilities for this evening and night on-call position are as follows:
Triage phone calls received after hours to provide phone support or home visits for current Hospice patients
Make home visits to provide care as appropriate
Admit patients to Hospice services (including associated documentation)
Coordinates and collaborates with the interdisciplinary team ensuring the enactment of the Plan of Treatment.

Education
- High School Diploma or equivalent
- ADN, BSN, MSN
- Current and valid Registered nurse license in the Commonwealth of Virginia or from a state that is part of a Compact agreement with Virginia
- CPR
- CHPN Certification preferred, but not required
- Requires one year of recent Medical/Surgical Experience
- Home health or hospice experience preferred, but not required
- Night Call
- Phone Triage. Respond to patient/family calls assuring the evaluation of the situation and appropriate level of response needed to meet patient/family needs for support and care
- Make home visits as needed to meet patient/family service requirements in accordance with physician’s orders and Care Plan goals, including admissions, routine visits, and symptom management visits.
- Make home visits to pronounce patients at time of death, support family and provide post-mortem care
- Completes 5 Rights with 100% compliance. Knowledge of medications used.
- Knowledge and use of high risk medications.
- Ability to perform calculations for mixtures/solutions.
- Reconciles patient medications and performs DRR.
- Updates plan of care based on continuing assessment of patient outcomes.
- Documentation reflects nursing interventions, patient responses, as well as follows through with patient outcomes.
- Evaluation of age specific instruction/teaching is demonstrated and documented through monitoring of the learning process, seeking feedback from patient and family, and return demonstration if appropriate.
- Assessments are complete and appropriate for patient’s age, special care needs, and patient's knowledge level, psychosocial, emotional, physical, and spiritual status.
- Reassessment is timely and performed as patient condition warrants.
- Documentation of assessment is timely.
- Communicates assessment data as necessary to interdisciplinary team members.
- Appropriate referrals are made with evidence of follow-up documentation in the medical record.
