LPN - ER

PURPOSE OF POSITION

To provide skilled home care under the direction and supervision of a registered nurse to patients to promote, restore and maintain the health of the individual through teaching, preventative and rehabilitative interventions.

 RESPONSIBILITIES & EXPECTATIONS

• Abides by and promotes agency policies and procedures, regulatory guidelines of CMS, ODH and the Joint Commission.
• Adheres to the scope of practice for licensed practical nurse per the Ohio State Practice Act.
• Works collaboratively with the case manager and other members of the multidisciplinary team to provide skilled nursing care based on the plan of care in a variety of settings.
• Utilize teach back to demonstrate and educate patients and caregivers.
• Provide case manager and other members of the team information and updates on patient condition and response to interventions. 
• Documents observations, interventions, significant finding and communication with other members of the care team in the patient's record.
• Completes assigned responsibilities in required time frame and accepts additional duties and responsibilities based on the needs of the patient/employee team and that of the agency
• Maintains average monthly productivity standard of 6 visits/8 worked hours.
• Participates in the agency's performance improvement plan.
• Utilizes all applicable computerized systems within the department. 
• Communicate requests, suggestions, concerns or problems to the appropriate personnel.
• Completes annual competency requirements.
• Complies with agency infection control and safety policies.
• Maintains a professional appearance.
• Maintains Aultman standards of confidentiality, including the HIPAA Privacy Rule.
• Creates and fosters a work environment that is consistent with the Mission, Vision and Values of the Aultman Health Foundation.
• Supports organizational strategic goals.
• Utilize the HRO Tools and Tones to promote patient safety.
• Abides by the Aultman Code of Conduct at all times.

QUALIFICATIONS

• Ohio Licensed Practical Nurse
• Two years of nursing experience preferred
• Able to work independently and accurately with attention to detail
• Excellent interpersonal and oral/written/telephone communication skills; strong organizational and problem-solving skills; ability to stimulate new ideas
• Demonstrate emotional intelligence, analytical and critical thinking skills, and problem-solving skills
• Organized with excellent time management skills
• Skilled and complete documentation
• Basic computer skills and typing skills
• Utilize the HRO Tool and Tones to promote patient safety

WORKING CONDITIONS:

• Hours of operation with shifts as assigned including occasional overtime or scheduled as necessary.
• Position is less than 40 hours/week.
• Rotates weekend, evenings and holidays with staff to facilitate adequate coverage for all patients.
• Subject to frequent interruptions and changes in priority of duties throughout the day.
• Sitting/standing/moving about during working hours (see attached Physical Requirements Addendum for details).
