SUMMARY
Maintain confidential per HIPAA regulations. Secure medical records in alphabetical and chronological order per the HealthWorks Program. Requests medical records for all youth coming into custody. Follow up an all medical records requests.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Maintain confidential per HIPAA regulations.
Requests all medical records from the providers that are in ICARE for the new cases.
Data enter the medical records requested (date requested, date received, what was requested and from where) into the HealthWorks in house system.
Send a second medical record request 2 weeks from the first request for the medical records that we did not receive.
Send the medical file for all transferred youth to the requesting county as well as send a consent for the youth that transfer to Cook County to retrieve the medical records.
Forward all ongoing medical records received to the caseworker, and the 0-5 year old medical records are sent to the Medical Case Management Agencies within as well.
Mail out the Health Passport to the caregiver once the youth has completed the Interim process.
Send the Administrative Case Reviews (ACR) packet provided by the Medical Case Management Agency for the 0-5 year old to the caseworker for the youths upcoming ACR on their case loads
Provide leadership and backup coverage to the Medical Records Supervisor, as required.
Archive ward’s charts as directed by policy.
Lift at least 30 lbs.
Replies to incoming requests in a timely manner.
File medical documents in a timely manner.
Assist with Quality Review.
Maintain a clean and organized file room.
Attend meetings as required.
Perform all other duties as assigned.
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
EDUCATION and/or EXPERIENCE
General Educational Diploma (GED) and/or one year related experience and/or training; or equivalent combination of education and experience.
LANGUAGE SKILLS
Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, staff providers, and other employees of the organization.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
COMPUTER SKILLS
Ability to data enter information and some knowledge in Microsoft excel, word, and outlook.
REASONING ABILITY
Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.
CERTIFICATES, LICENSES, REGISTRATIONS
Valid driver's license.
Valid vehicle insurance.
