Job Description: Are you an experienced Hospice Case Manager looking for a rewarding contract position? We are seeking a compassionate and skilled Home Care Hospice Case Manager for a 12-week contract in Colorado Springs, CO. Enjoy a Monday through Friday schedule with no weekends and earn up to $57 per hour while making a significant impact on patients' lives.
Responsibilities:
- Manage and coordinate hospice care for patients in their homes.
- Develop and implement individualized care plans.
- Provide compassionate support and education to patients and their families.
- Collaborate with interdisciplinary team members to ensure comprehensive care.
- Document patient progress and adjust care plans as needed.
- Travel to patients' homes using reliable transportation.
Requirements:
- Minimum of 2 years of experience in hospice care.
- Current RN license in good standing.
- Reliable transportation for home visits.
- Strong clinical skills and ability to manage complex care needs.
- Excellent communication and interpersonal skills.
- Compassionate and patient-centered approach to care.
