Position : Home Care Hospice Case Manager
Location : Colorado Springs, CO
Duration : 12 weeks
Hours : Monday through Friday, 8 AM - 5 PM (No weekends)
Pay Rate : Up to $57 per hour
Job Description : Are you an experienced Hospice Case Manager looking for a rewarding contract position? We are seeking a compassionate and skilled Home Care Hospice Case Manager for a 12-week contract in Colorado Springs, CO. Responsibilities :
- Manage and coordinate hospice care for patients in their homes.
- Develop and implement individualized care plans.
- Provide compassionate support and education to patients and their families.
- Collaborate with interdisciplinary team members to ensure comprehensive care.
- Document patient progress and adjust care plans as needed.
- Travel to patients' homes using reliable transportation.
