Job Description:
The selected candidate will work with patients who have various physical and cognitive impairments, providing them with the necessary skills to perform daily activities and achieve independence.
- Responsibilities:
- Conduct thorough assessments of patients' needs and abilities.
- Develop and implement individualized treatment plans.
- Collaborate with other healthcare professionals to ensure comprehensive care.
- Communicate effectively with patients, families, and staff.
Requirements:
- Current Illinois license (or willingness to obtain)
- CPR/BLS certification
- NPI number
- Ability to lift 50-100lbs
Working Conditions:
This is an inpatient Rehab/Acute level position. The working hours are Monday through Friday, 8am-4:30pm, with some weekend shifts possible.
