Come join our team of professionals at Aurora Behavioral Health System West. This 100-bed, free-standing psychiatric inpatient hospital and outpatient program located in Glendale, Arizona is looking for an experienced Discharge Planner. Our psychiatric facility is licensed to treat adolescents and adults for a wide range of psychiatric conditions and chemical dependency.

The basic purpose of this position is to provide quality case management and coordinate care for discharging patients. This includes working directly with outpatient providers, programs, and agencies to assist patients in the development of an appropriate discharge plan.

Responsibilities include:
- Participates as a member of a multi-disciplinary team by collaboratively engaging with the clinical team regarding concerns or barriers to discharge plan.
- Responsible for documenting each engagement with the patient and anyone involved in the patient's care.
- Coordinates patient's staffing for Aurora and clinic treatment teams by providing resources and discharge service solutions.
- Manages assigned discharging patients with appropriate discharge plans in place and communicate discharge plans to external agencies.
- Schedules follow up appointments and submit referrals to appropriate level of care facilities.
- Assists with the process to have the patient step down to PHP or IOP upon discharge.
- Scheduling family sessions, CFT's, and or outpatient treatment team meetings with assigned clinics and confirm a safe discharge.
- Develops and coordinates an individualized discharge plan for the patient by utilizing treatment team and chart information to determine the patient's aftercare needs.
- Identifies and contacts family or community resources such as, group homes, or mental health practitioners to meet patient's aftercare needs.
- Timely and accurate documentation of treatment provided to patient in medical record.
- Maintains effective working relationships and communications with internal and external customers.
- Assists in finding solutions to utilization review problems.
Requirements:
- 2 years of case management experience
- 2 years of behavioral health experience
- Excellent customer service skills
- Must be able to multitask in a fast paced environment
- Must be able to pass a drug screen this is a safety sensitive position
- Must be able to obtain a fingerprint clearance card
