Position Summary: The Nurse Case Manager plays a pivotal role in managing the care plans of injured workers, ensuring effective communication among medical professionals, clients, patients and their families, and insurance adjusters. This position is ideal for a self-motivated individual passionate about patient care and dedicated to facilitating successful return-to-work outcomes. The role offers the autonomy to work independently while being part of a growth-oriented team.


Key Responsibilities:

  • Care Coordination: Develop and implement individualized care plans for injured workers, coordinating with healthcare providers to ensure appropriate treatment and rehabilitation.
  • Communication: Serve as the primary liaison among all stakeholders, including medical professionals, clients, patients and their families, and insurance adjusters, to facilitate seamless information flow and collaborative decision-making.
  • Monitoring Progress: Regularly assess and document patient’s progress, adjusting care plans as necessary to promote optimal recovery and timely return to work.
  • Education and Support: Provide education to patients and their families regarding treatment plans, expected outcomes, and strategies to prevent re-injury. Offer support and guidance throughout the recovery process.
  • Compliance and Documentation: Ensure all case management activities comply with legal, regulatory, and ethical standards. Maintain accurate and up-to-date records in accordance with company policies and industry best practices.


Qualifications:

  • Education: Associate or Bachelor's degree in Nursing from an accredited institution.
  • Licensure: Current Registered Nurse (RN) license in the state of practice.
  • Experience: Minimum of three years of clinical nursing experience, preferably in case management, occupational health, or a related field. Experience in the workers compensation industry is highly desirable.
  • Skills: Strong organizational and time-management abilities. Excellent interpersonal and communication skills. Proficiency in using case management software and other relevant computer applications.
  • Attributes: Self-starter with the ability to work independently. Comfortable with challenges and committed to staying at the forefront of industry innovations. Team player with a growth mindset and a passion for delivering the highest standard of care.


Work Environment:

  • Location: Primarily a remote position, offering the flexibility to work from home.
  • Schedule: Monday through Friday, standard business hours. No shift work required.
  • Travel: Some travel may be necessary for attending medical appointments and meetings with clients or patients.
