Job Description

Job Title : Home Health Occupational Therapist

Job Specialty : Operations - Home-Hospice Care Operations

Job Duration : 13 weeks

Shift : 8hr Day Shift (40 hours per week)

Guaranteed Hours : Not specified

Experience : 3 years of occupational therapy experience in skilled nursing facility, home health, or acute care (home health preferred)

License : Active Occupational Therapist license in Wisconsin

Certifications : Basic Life Support (BLS) from the American Heart Association

Must-Have :

Bachelor's degree in Occupational Therapy
Proficient in assisting with activities of daily living (ADLs) and adaptive equipment
Reliable transportation, active driver's license, and auto insurance

Job Description :

Provide home health occupational therapy services covering Kenosha and Racine counties.

Patients primarily include adults and geriatrics dealing with post-stroke, post-joint replacement, weakness, deconditioning, COPD, CHF, etc.

Conduct evaluations and standard visits, with an expectation to see 6 patients or points per day (Evaluations are 1.5 points, standard visits are 1 point).

Collaborate effectively with other disciplines such as nursing and physical therapy.

Initial orientation at the Milwaukee office, followed by additional orientation with a preceptor.

Dress code is business casual or any solid color scrubs (must maintain a professional appearance).

Preferences :

Experience with EPIC software is highly preferred but not required.

Interview Process :

Initial pre-screen interview followed by a second-round interview with the manager over the phone.
