What are the responsibilities and job description for the LVN Field Nurse PRN Dallas, TX position at AURORA Home Health?
As a licensed LVN, you will visit patients in their homes and provide nursing care for them under direction of RN's and in compliance with the physicians' orders and CMS. Your specific duties for this role will include but not limited to:
- Provide professional nursing services under the direction of a RN in compliance with the state’s Nurse Practice Act, any applicable licensure/certification requirement, applicable healthcare standards, governmental laws and regulations, and company policies and procedures..
- Utilize a systematic, individualized goal/outcome driven approach in implementing the nursing plan of care.
- Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team in a timely manner regarding patient/family needs and status changes. Participate in care coordination activities/ case conferences and discharge planning.
- Complete a clinical note for each visit within required time frames, capturing physical assessment, home environment, medication changes and any skilled interventions provided during the course of the visit as well as any care coordination with health care team. This note will be incorporated into the patient’s record
Qualifications:
- Must be a graduate of an approved school of nursing and licensed as a Licensed Vocational/Practical Nurse in the state of employment
- Must possess a valid state driver’s license, automobile liability insurance, and have dependable transportation
- Must have a minimum of one year of clinical experience
- Previous adult home health experience is required
- Axxess software knowledge is required
- IV certification/experience required
