AURORA Home Health has an immediate opening for a full-time QA - LVN Home Health position. The right candidate MUST HAVE Home Health, QA experience and Axxess Software knowledge. Must be someone who is a self starter and enjoys working in a fast-paced environment.
- Staffs new referrals to appropriate clinician based upon location of the patient and skill of the clinician.
- Oversees and implements quality assurance activities to improve clinical and patient care practices.
- Provides assistance to other administrative roles as needed.
- Notifies billing and authorizations staff of non-Medicare patients and their scheduled needs.
- Abstracts the necessary information as stated by the physician or other health care providers from the medical record and assigns the appropriate diagnosis codes.
- Verifies patient information is correct and follow-up as necessary to obtain correct information.
- Ensures that the primary diagnosis and secondary diagnoses reflect the care and treatment to be rendered.
- Develops and implements quality assurance protocols and policies.
- Monitors and reviews clinical processes to ensure compliance with state, federal, and healthcare industry regulations.
- Conducts regular audits of patient care records to identify areas for improvement.
- Ensures compliance with clinical guidelines, procedures, and standards.
QUALIFICATIONS:
- Home Health experience is required
- Ability to work in fast pace environment and problem solve is critical.
- Must communicate effectively in person, via phone and written formats.
- Proficient knowledge of clinical computer software and Microsoft software.
