Author Health provides comprehensive outpatient psychiatric treatment to seniors and adults with serious mental illness (SMI), substance use disorders (SUD), and dementia. Our integrated care team includes MDs, NPs, LCSWs, and RN case managers working alongside primary care providers and community organizations. We work hand-in-hand with patients and their caregivers to make mental healthcare and services accessible from a patient's home.

Pay Ranges
$60,000—$70,000 USD
The Community Outreach Manager conducts direct patient outreach to increase patient engagement, coordinating between healthcare providers and patients/caregivers to overcome barriers to care. This role combines clinical care coordination with relationship building to improve patient health outcomes and quality of life.
WHAT IS YOUR SUPERPOWER?
- Conduct in-person outreach primarily at healthcare facilities and PCP offices to enroll and engage eligible patients, and at patients homes, as needed
- Build and maintain relationships with healthcare partners (PCPs, administrators, case managers) to generate referrals to Author Health providers
- Drive clinical engagement by meeting monthly targets and ensuring completion of appropriate care
- Assess and address barriers to care through creative problem-solving and resource coordination
- Educate providers, patients, and caregivers about available services and benefits
- Plan and optimize daily travel routes across your service area
- Maintain accurate and timely documentation of all patient interactions and outcomes
- Required: Current or former RN, LCSW, LMFT, or LMHC license, inclusive of completed practicum hours; -or- Master's Degree in Social Work, or related field
- Required: Excellent time management abilities
- Required: Valid driver's license and reliable transportation; ability to travel up to 100 miles round trip daily (reimbursed)
- Required: Located in Irving, TX; knowledge of the Irving market
- Required: Excellent time management abilities; cultural competency and sensitivity
- Required: Strong computer literacy and documentation skills
- Preferred: 3 years of experience in health care working with patients and caregivers with chronic medical conditions
- Preferred: 3 years experience with mental health, substance abuse disorders, and dementia patients
- Preferred: 3 years experience in customer service/sales, case management, and/or with cold outreach experience
- Preferred: Bilingual (Spanish-English)
- Knowledge of healthcare systems and community resources
- Demonstrated success in relationship building and stakeholder management
- Strong interpersonal skills
- Problem-solving and crisis management experience
