AuthoraCare Collective is currently seeking a PRN Home Based Primary Care Nurse Practitioner position for our Integrated Health Services (Skilled home-based-care) department supporting our High Point, Kernersville and Winston-Salem service areas. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area. Assume on call responsibilities as scheduled.
The Home Primary Care Nurse Practitioner (NP) is responsible for delivering comprehensive primary care, palliative care, and chronic disease management to patients in their homes. This role includes conducting physical examinations, diagnosing and treating illnesses, managing chronic conditions, engaging in advanced care planning discussions, and coordinating care with other healthcare providers.
The NP focuses on health promotion, disease prevention, and patient education to ensure continuous and holistic care for home-bound patients. Building and maintaining therapeutic relationships with patients and caregivers, including providing emotional support and education, is a key aspect of this role. The NP will collaborate closely with the interdisciplinary team and effectively communicate, both verbally and in writing, with team members, patients, caregivers, physicians, pharmacists, and other healthcare professionals and community service organizations. Participation in on-call coverage for the Integrated Health Services population is required as assigned.
