What are the responsibilities and job description for the Occupational Therapist position at AuthoraCare Collective?
AuthoraCare Collective is currently seeking an Occupational Therapist for our Integrated Health Services (Skilled home-based-care) department. This is a full-time position Monday - Friday 8:00am - 5:00pm.
This position will support our Greensboro, High Point, Kernersville and Winston-Salem service areas. Must be able to travel to, enter, and function in the homes of any patient within the service area.
The Occupational Therapist provides services to restore function, improve mobility, relieve pain, prevent, or limit permanent physical disabilities of patients suffering from injuries and disease. Manage the therapy Plan of Treatment for a case load of Integrated Health patients in collaboration with other members of the interdisciplinary team (IDT). Assess and identify patient problems, establish therapy goals, and interventions.
Work with physician to establish a treatment plan using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease pain and help increase mobility and endurance. Establish and maintain therapeutic relationship with patient, including providing community support for resources and education. Therapist may evaluate and treat hospice patients as needed. Document the goals and outcomes of care.
