AuthoraCare Collective is currently seeking a PRN Kids Path Physician for our Greensboro service areas.
Job Summary:
Assist in the administration of the medical component of the patient care programs for Kids Path. Serve as a medical resource to assigned interdisciplinary teams by providing hospice and palliative care expertise. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area to assess and provide care to Hospice patients.
Participates in direct/clinical care/management of Kids Path patients. Provide home visits for symptom management, determination of prognosis if needed. Hospital visits, if needed, to ensure the continuation of the plan of care. Assists in administrative aspects of care including referral management, prescription management, and other care coordination tasks. Provides Home health and perinatal consultation, as requested. Participates on assigned patient care committees. Participates in the patient care on call system. Occasionally, may serve as primary attending physician for a hospice patient.
