AuthoraCare Collective is currently seeking a Hospice RN/LPN Clinical Triage Center Nurse. This position is Part-time (20 hours per week) with a flexible work schedule; Monday - Friday 8:00am - 5:00pm. This is a remote position; must reside in the Greensboro, NC or Burlington, NC area and you must be able to travel, if needed, during crisis staffing to enter, and function in the homes of any patient/caregiver within the service area.
Job Summary:
Coordinates timely communication with provider practices, pharmacies, vendors, laboratories, and patient/family. Receives requests from care managers for follow up with; providers for new orders, reporting abnormal patient findings, medication renewal orders. Reviews after hours report to follow up on any patient concerns. Assists with tuck in calls.
In conjunction, works alongside clinical call to support to the patient care line and covers clinical call during lunch and absences. Provides patient-focused, effective direction and customer service-oriented responses to incoming and outgoing calls. Maintains ability handle timeliness of patient calls, responsiveness to ensure patient care needs are being met.
