AuthoraCare Collective is currently seeking an RN On-call Weekend (Integrated Health Services, Home Health and Hospice). This is a Full-time position: Friday, Saturday and Sunday 10:00am - 8:30pm.
Job Summary:
Triages incoming calls for patients on Integrated Health Services. Assess and identify patient problems and respond appropriately with clinical intervention. Complete in-home clinical visits as applicable to assess and treat patient. Coordinate with appropriate resources to meet patient’s after-hours needs. Work closely with members of the care teams to ensure quality communication and patient care. Schedule and add appropriate documentation to EMR with each patient interaction including obtaining physician orders. Complete assigned EMR workflow.
Establish and maintain therapeutic relationship with patient, including providing disease education, medication management, clinical care and end of life care. Will send communication to appropriate discipline to ensure continuity of care.
