What are the responsibilities and job description for the LPN/RN position at Autumn Behavioral Health Center?
SUMMARY:
The Nurse is a member of the Treatment Team. S/he is responsible for coordinating medical care for program participants and reports directly to the Medical Director and Medical Providers. This is an hourly position with hours and schedule established to meet client needs.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Ensures that general health assessments are completed for all patients.
2. Facilitates referrals to physicians, dentists, and other health care professionals as indicated by the assessed needs of each patient.
3. Provides assistance to personnel in determining the appropriateness for admissions as needed.
4. Participates in clinical staffing meetings.
5. Conducts groups and other educational groups as necessary.
6. Maintains appropriate patient health records.
7. Interacts with community agencies and refers patients to facilitate their health maintenance.
8. Provides medication protocols, detoxification protocols, and drug screening procedures.
9. Conducts medical histories and physicals as well as additional medical evaluations as directed by the physician and agency protocols.
10. Is available to other staff to address / answer concerns, provide after-hour directives and help problem solve patient related issues.
11. Insures the flow of information both horizontally and vertically within the organization.
12. Provides appropriate documentation for contract and system requirements and clinical best practice.
13. Makes sound, independent, cooperative and ethical decisions.
14. Assists the physician in direct client care.
ADDITIONAL DUTIES AND RESPONSIBILITIES:
Occasional travel to other locations.
Performs other duties as deemed appropriate by supervisor.
REQUIRED QUALIFICATIONS:
1. The Nurse must have a minimum of a current Ohio RN/LPN license.
2. Must have current First Aid/CPR certification.
3. S/he must possess and demonstrate effective organizational skills.
4. S/he should demonstrate an extensive background in working with substance abuse clients.
ESSENTIAL KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS:
1. Must demonstrate the ability to effectively function as a Autumn Treatment Center, LLC team member.
2. Must demonstrate the ability to effectively manage multiple tasks concurrently.
3. Must possess and demonstrate exceptional communication and organizational skills.
4. Must demonstrate the ability to customarily and regularly exercise discretion and independent judgment in significant matters.
5. Must demonstrate the ability to relate with empathy and sensitivity to staff, clients and the community.
6. Must demonstrate direct experiential knowledge of mental health and/or addiction medicine.
CERTIFICATIONS, LICENSES AND REGISTRATIONS:
The minimum of a current Ohio Registered Nurse (RN) license or Licensed Practical Nurse (LPN) is required for this position.
