Purpose: Responsible to coordinate and implement an activities program that meets the needs and interests of the residents of the facility, while working with other members of the interdepartmental team to ensure all resident needs are met. Provides transport assistive services to assigned residents in accordance with care plans, facility policies and procedures and at the direction of supervisor(s).
Essential Functions- Job Duties: The following duties are considered essential to the job. Qualified individuals must have the ability, with or without reasonable accommodation, to perform the following duties:
- Plans, organizes, and implements daily calendar activities, with assistance and feedback from residents, to provide for a structured therapeutic environment.
- Adheres to all facility and governmental regulations documenting attendance, progress and all pertinent information. Maintains accurate records of activity participation, hydration, etc., utilizing the appropriate forms. Completes activity assessments for residents as needed.
- Develops a base of knowledge about each resident. From this knowledge: plans, schedules and implements group activities with residents as appropriate.
- Arranges and takes residents to various outings out in the community.
- Ensures building pet(s) are fed and areas are kept clean.
- Assists with basic care needs of residents as needed, including, but not limited to, serving meals, hydration needs, and transportation of residents and general housekeeping/organization of activity areas.
- Works a flexible schedule to meet the needs of the activities department.
- Ensure proper resident documentation is completed in Point Click Care.
- Promotes and encourages successful outcomes of resident activities, both group and individual.
- Provides transportation services to residents as scheduled or as needed for appointments or events.
- Assists resident with transfer activities during the transport in accordance with care plans and established policies and procedures.
- Assists resident with onboarding and offboarding from vehicle including use of mechanical lifts, turning, moving, positioning, and transporting into and out of the vehicle and into suitable wheelchair when applicable.
- Safeguards resident medical paperwork as needed during medical transports. Provides paperwork to receiving provider.
- Accepts directions from the Life Enrichment Director, Executive Director, or designee to coordinate transport schedule.
- Delivers resident timely for appointments and events.
- Completes documentation daily as assigned.
- Follow all applicable state regulations.
Education & Knowledge Requirements- Specific education, certification, and licensure:
- High School Diploma or equivalent
- Valid, unencumbered State issued Driver’s License
- Must be at least 23 years of age
- Ability to communicate In English, both verbally and in writing, in a clear and concise manner
Essential Job-Related Experience/Abilities:
- The ability to lift, bend, stoop, push, pull or transport residents, supplies or furniture up to 50 pounds; may ask for assistance if needed. Must be able to perform job duties without restrictions.
Preferred Job-Related Experience/Abilities/Education:
- Two (2) years’ experience planning/organizing activity programs
- One (1) year demonstrated knowledge of the methods and techniques of motivating the elderly
- Minimum of five (5) years' experience interacting with seniors
