Job Description: Patient Care Advocate
We are seeking a dedicated and compassionate Patient Care Advocate to join our team at Autumnwood of Deckerville.
Key Job Functions
- Take and record vital signs under the direction of registered nurses or licensed practical nurses.
- Assist with measuring and recording guest height, weight, intake, and output.
- Maintain a clean and safe environment for guests.
- Support guests' personal care needs, including bathing, grooming, and toileting assistance.
- Help ensure guests can eat and hydrate properly.
- Monitor guests' skin health and report any changes to licensed nurses, providing necessary skin care.
- Meet guests' mental health and social service needs.
