Job Summary
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including acute care and specialized units such as PICU and Level I Trauma Centers. The LVN will work closely with patients, families, and healthcare professionals to ensure optimal health outcomes and support for individuals with diverse needs.
Duties
- Administer medications and treatments as prescribed by physicians
- Conduct patient assessments to monitor health status and progress
- Assist in discharge planning to ensure smooth transitions for patients
- Provide health coaching and education to patients and families regarding care plans
- Perform tube feeding and care for patients requiring specialized nutrition
- Care for infants and children, ensuring their safety and comfort during treatment
- Collaborate with multidisciplinary teams to enhance patient care strategies
- Support individuals with disabilities by adapting care approaches to meet their unique needs
- Document patient information accurately in electronic health records
Skills
- Proficient in patient assessment techniques and acute care protocols
- Experience in providing care within a Level I Trauma Center or similar environment
- Knowledge of health coaching principles to empower patients in managing their health
- Ability to perform dialysis procedures when required
- Familiarity with infant care practices, including developmental needs and safety protocols
- Strong communication skills for effective collaboration with healthcare teams and families
- Competence in discharge planning processes to facilitate patient transitions from hospital to home or other facilities
- Understanding of tube feeding administration and monitoring techniques
