Job Summary
We are seeking a compassionate and dedicated Hospice Nurse to join our team. The ideal candidate will provide high-quality nursing care to patients with terminal illnesses in a home care setting, ensuring comfort and dignity during their final stages of life. This role requires a strong commitment to patient-centered care, as well as the ability to work collaboratively with families and interdisciplinary teams.
Responsibilities
- Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Administer medications and treatments as prescribed, including catheterization and spinal taps when necessary.
- Provide education and support to patients and families regarding end-of-life care options, including family planning and symptom management.
- Monitor patient progress and adjust care plans as needed to ensure optimal comfort.
- Perform triage duties to prioritize patient needs effectively.
- Document all patient interactions and care provided in compliance with established standards.
- Participate in interdisciplinary team meetings to discuss patient care strategies and outcomes.
- Travel to patients' homes for visits, providing compassionate support in a familiar environment.
Qualifications
- Valid nursing license in the state of practice.
- Experience in home care or hospice settings preferred.
- Knowledge of pediatric care is a plus, particularly for families with developmental disabilities.
- Familiarity with travel nursing practices is beneficial.
- Ability to perform specialized procedures such as botulinum toxin treatment is desirable.
- Strong communication skills, both verbal and written, with an emphasis on empathy and active listening.
- Excellent organizational skills with attention to detail in documentation and patient care standards.
- Ability to work independently while being part of a larger healthcare team.
