Overview
We are seeking a compassionate and dedicated Registered Nurse - Home Health to join our team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being while promoting independence. You will work closely with patients, families, and healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities
- Conduct comprehensive assessments of patients' health status and needs in a home setting.
- Develop and implement personalized care plans based on assessment findings and patient goals.
- Administer medications and treatments as prescribed, ensuring adherence to safety standards.
- Provide education and support to patients and families regarding health management, including discharge planning.
- Monitor patients for changes in condition and respond appropriately to emergencies.
- Collaborate with interdisciplinary teams to coordinate care and ensure continuity of services.
- Maintain accurate documentation of patient care activities, progress, and outcomes in compliance with regulatory standards.
Skills
- Strong knowledge of physiology and nursing principles related to home health care.
- Experience in ER or primary care settings is preferred for managing acute situations effectively.
- Proficiency in sterile processing techniques to ensure patient safety during procedures.
- Ability to perform catheterization and suture as needed for patient care.
- Familiarity with hospice care practices to provide compassionate end-of-life support.
