Job Summary
We are seeking a compassionate and skilled Hospice Nurse to provide high-quality care to patients in their final stages of life. The ideal candidate will possess a strong commitment to patient comfort and dignity, ensuring that individuals receive the emotional and physical support they need. As a Hospice Nurse, you will work closely with patients, families, and interdisciplinary teams to deliver comprehensive care tailored to each patient’s unique needs.
Duties
- Conduct thorough patient assessments to determine care needs and develop individualized care plans.
- Monitor vital signs and manage symptoms effectively, ensuring patient comfort.
- Administer medications and treatments as prescribed, including tube feeding and venipuncture.
- Provide triage services for patients experiencing acute changes in condition.
- Collaborate with physicians and other healthcare professionals in the management of infectious disease care.
- Educate patients and families about end-of-life care options, including emotional support resources.
- Document all patient interactions and care provided in compliance with ICD-9 coding standards.
- Participate in regular team meetings to discuss patient progress and adjust care plans as necessary.
Requirements
- Valid nursing license in the state of practice.
- Proven experience in acute care settings; hospice or palliative care experience is preferred.
- Proficiency in patient assessment techniques and vital signs monitoring.
- Knowledge of cardiac catheterization procedures is a plus.
- Ability to perform dialysis procedures if required by specific patient needs.
- Strong interpersonal skills with the ability to communicate effectively with patients and families during sensitive times.
- Familiarity with managing infectious diseases within a hospice setting is advantageous.
- Must be compassionate, empathetic, and able to work collaboratively within an interdisciplinary team.
