Job Title: Medication Program Manager / Assistant Assisted Living Program (ALP) Coordinator
Location: New York State Department of Health Licensed Assisted Living Facility in Wappingers Falls, NY (Dutchess County)

Position Overview: We are seeking a highly motivated, compassionate and organized individual to serve as our Medication Program Manager / Assistant ALP Coordinator. This dual-role position is crucial in ensuring the well-being of our residents by managing their medication needs and assisting in the coordination of our Assisted Living Program. The ideal candidate will have a strong background in healthcare, excellent organizational skills, and a passion for working with seniors.
Key Responsibilities:
· Oversee the medication management program, including the assistance of resident self-administration, documentation, and monitoring of medications for residents.
· Collaborate with healthcare providers, pharmacists, and families to ensure accurate and timely medication delivery.
· Conduct regular medication audits and reviews to ensure compliance with state regulations and facility policies.
· Assist the ALP Coordinator in developing and implementing individualized care plans for residents.
· Coordinate and participate in resident care meetings, providing input on medication-related issues.
· Train and supervise staff on medication administration procedures and best practices.
· Maintain accurate and up-to-date records of medication administration and resident health status.
· Respond to medication-related emergencies and provide appropriate guidance.
· Support the ALP Coordinator in organizing and facilitating resident appointments and follow up.
· Foster a positive and supportive environment for residents, staff, and families.
Qualifications:
· Minimum of 2 years of experience in medication management and/or assisted living program coordination.
· LPN license is preferred, but not required. Strong organizational skills, attention to detail and communication skills are required.
· Ability to work collaboratively with a multidisciplinary team.
· Compassionate and patient-centered approach to care.
· Proficiency in electronic health records (EHR) systems and computerized programs.
