About the Role:
Avalon Center is looking for a dedicated and compassionate Transitional Housing Case Manager. You'll support survivors of domestic violence in finding stable housing and becoming self-sufficient.
What You’ll Do:
- Housing Advocacy:
- Conduct assessments and intake for the Transitional Housing Program.
- Develop Housing Stability Plans with survivors.
- Help survivors find and move into housing.
- Case Management:
- Assist survivors with goal setting and accessing services.
- Act as a liaison with other service providers.
- Provide legal, medical, systems advocacy, crisis intervention, safety planning, and more.
- Keep detailed records of all client interactions.
- Community Collaboration:
- Build relationships with local officials, agencies, landlords, and utility companies.
- Promote the Transitional Housing Program to landlords.
What We’re Looking For:
- High school diploma or GED required; a bachelor’s degree or relevant experience is a plus.
- Available for on-call rotation.
- Quick and effective crisis response skills.
- Strong communication and advocacy skills.
- Team player with a commitment to ending power-based violence and understanding of trauma-informed care.
- Reliable vehicle, valid driver’s license, and current insurance.
- Ability to handle physical tasks like standing, climbing stairs, driving, and lifting up to 25 lbs.
