RN Case Manager Hospice **Must have at least 12 months of hospice in home settings experience within the past 3 years. HCHB and Hospice experience required, Saturdays and Sundays. Guaranteed 16 up to 24 hours per week (allowing any extended documentation time needed). Mix of on call, scheduled visits, admissions, and emergent visits that arise. Would cover both Milford, CT and surrounding areas
