As a CNA Supervisor you are the front line of the Clinical Department. Your work will ensure our residents receive the high standard of care they have grown to expect at  Avantara Lincoln Park  by conducting 1 on 1 orientations and trainings with new CNA's, motivating and providing guidance to new CNA’s, performing Activities of Daily Living, observing changes in condition, ensuring a safe environment, and maintaining education. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

Essential Functions

Reasonable accommodations may be made to enable individuals with disabilities to perform

1. Provides general supervision of the Certified Nurse Assistant’s.

2. Assures that subordinate staff member’s tasks are completed on time and checks the

quality of work.

3. Completes annual performance reviews of all subordinate staff.

4. Provides guidance and education to staff in regards to their performance.

5. Provides counseling to subordinate staff members when needed.

6. Assists with scheduling of C.N.A’s by finding replacements when staff members call off if

needed.

7. Acts as a back-up C.N.A. during staffing shortages or when directed to do so.

8. Performs various tasks assigned by the floor nurse including checking vital signs,

weighing Guests, applying creams / ointments and collecting specimens.

9. Must be knowledgeable of individual care plans and support the care planning process

by providing supervisors with specific information and observations of the Guest’s

needs, preferences and report any behavioral changes.

10. Completes medical records documenting care provided and other information in

accordance with nursing policies while maintaining strict confidentiality.

11. Assure that established infection control and prevention practices and standard

precautions are maintained at all times.

12. Follow established safety precautions when preforming tasks and using equipment and

supplies.

13. Assists with the training of new staff members.

14. Reports all hazardous conditions, damaged equipment, accidents / incidents and supply

issues to the supervisor.

15. Maintains the comfort, privacy and dignity of Guests and interacts with them in a

manner that displays warmth, respect and promotes a caring environment.

16. Ensure each Guest receives person centered care.

17. Must answer and respond to call lights promptly and courteously.

18. Assist with new admissions, re-admissions and assist with the transfer of Guests to

different rooms within the facility.

19. Communicates and interacts effectively and tactfully with Guests, visitors, families,

peers and supervisors.

20. Attend and participate in departmental meetings and in-services as directed.

21. Recognize your role as part of the Quality Assurance and Performance Improvement

QAPI) efforts of your organization.

22. Attend trainings to build understanding and capacity to undertake QAPI work.

23. Carry out QAPI roles and responsibilities as assigned.

24. Follow established policies and procedures in support of QAPI efforts.

25. Any noted opportunities for improvement will be shared with leadership in the

organization.

26. Communicate to leadership upon witnessing a positive outcome of a QAPI project or

detecting barriers preventing project success.

27. Support QAPI efforts both verbally and non-verbally (i.e., via actions and attitude),

including adjusting performance and practice in accordance with QAPI initiatives and

findings.

28. Participate in compliance trainings as scheduled and adhere to compliance plan.

Supervisory Responsibility

This position provides general supervision to C.N.A.’s and other subordinate staff.

Work Environment

This job operates in a health care setting. This role requires regular walking to various locations

around the facility while pulling or pushing carts. This position works mostly in the Guest care

areas as well as in occasional outside weather conditions. This role may also come into contact

with patients who may have contagious illnesses.

Physical Demands

While performing the duties of this job, the employee is regularly required to talk and hear. This

position is very active and requires standing, walking, bending, kneeling, and stooping on a

regular basis. The employee must be able to lift and move items and patients weighing at least

50 pounds. Specific vision abilities required by this job include close vision, distance vision, color

vision, peripheral vision, depth perception and ability to adjust focus.

Position Type and Expected Hours of Work

This position is part of a health care facility that is open 24 hours a day, 365 days a

year. Rotating shifts as well as holiday and work during inclement weather will be required.

Travel

No travel is expected for this position.

To be eligible for consideration applicants should have :

As a minimum, an unencumbered State of  Illinois  C.N.A. certificate and C.P.R. Certification. One (1) year of C.N.A. experience is preferable, but we are willing to invest in the right candidate.
