Summary / Objective
In keeping with our organization’s goal of improving the lives of the Guests we serve, the
Restorative Nurse plays a critical role in providing superior customer service and nursing care to
all Guests. Under the direction of the Director of Nursing, the Restorative Nurse LPN
implements and directs the facility’s restorative nursing program with the goal of helping
Guests reach and maintain their full mobility potential.
Essential Functions

1. Develops, implements, directs and evaluates the facility’s Restorative Nursing Program.
2. Meets and consults with the facility’s interdisciplinary team on a regular basis to
develop and maintain restorative care standards.
3. Ensures that the restorative nursing program complies with applicable laws, regulations,
and national restorative nursing standards and requirements.
4. Participates in the admission process by meeting with new Guests and providing them
with a clear overview of the restorative program.
5. Participates in discharge planning, development and implementation of Guest care
plans and assessments.
6. Performs administrative tasks such as charting, care planning, reports and etc.
Completes assigned MDS portions accurately and on time.
7. Assists with the recruitment and selection of restorative nursing staff.
8. Provides supervision to C.N.A’s and all subordinate staff which includes checking their
work to ascertain that assignments have been completed.
9. Completes annual performance reviews of all subordinate restorative staff. Provides
guidance and education to staff in regards to their performance.
10. Provides counseling and disciplinary action to subordinate staff members as needed.
11. Assists with and participates in the disciplinary process of subordinate restorative staff.
12. Assign all C.N.A’s and other subordinate staff to Guests. Assign break times and all
regular and special assignments.
13. Performs other nursing care assignments by working as a floor nurse when needed.
14. Only practices nursing procedures and tasks with the scope of licensure.
15. Ensure that Guest care plans are being followed and asses each Guest’s status in
accordance with their care plan.
16. Round with physicians when needed.
17. Must be knowledgeable of individual care plans and support the care planning process
by reporting specific information and observations of the Guest’s needs, preferences
and report any behavioral changes.
18. Assure that established infection control and prevention practices and standard
precautions are maintained at all times.
19. Follow established safety precautions when performing tasks and using equipment and
supplies.
20. Reports all hazardous conditions, damaged equipment, accidents / incidents and supply
issues to appropriate persons.
21. Maintains the comfort, privacy and dignity of Guests and interacts with them in a
manner that displays warmth, respect and promotes a caring environment.
22. Ensure each Guest receives person centered care.
23. Answer and respond to call lights promptly and courteously.
24. Communicates and interacts effectively and tactfully with Guests, visitors, families,
peers and supervisors.
25. Attend and participate in departmental meetings and in-services as directed.
26. Recognize your role as part of the Quality Assurance and Performance Improvement
QAPI) efforts of your organization.
27. Attend trainings to build understanding and capacity to undertake QAPI work.
28. Carry out QAPI roles and responsibilities as assigned.
29. Follow established policies and procedures in support of QAPI efforts.
30. Any noted opportunities for improvement will be shared with leadership in the
organization.
31. Communicate to leadership upon witnessing a positive outcome of a QAPI project or
detecting barriers preventing project success.
32. Support QAPI efforts both verbally and non-verbally (i.e., via actions and attitude),
including adjusting performance and practice in accordance with QAPI initiatives and
findings.
33. Participate in compliance trainings as scheduled and adhere to compliance plan.
Supervisory Responsibility
This position directly supervises Nursing department staff based on scope of practice.
Work Environment
This job operates in a health care setting. This role requires regular walking to various locations
around the facility while pulling or pushing carts. This position works mostly in the Guest care
areas as well as in occasional outside weather conditions. This role may also come into contact
with patients who may have contagious illnesses.
Physical Demands
While performing the duties of this job, the employee is regularly required to talk and hear. This
position is very active and requires standing, walking, bending, kneeling, and stooping on a
regular basis. The employee must be able to lift and move items and patients weighing at least
50 pounds. Specific vision abilities required by this job include close vision, distance vision, color
vision, peripheral vision, depth perception and ability to adjust focus.
Position Type and Expected Hours of Work
This position is part of a health care facility that is open 24 hours a day, 365 days a
year. Rotating shifts as well as holidays and work during inclement weather will be required.
