About the Role
We are seeking an experienced RN to join our team as a Case Manager. In this role, you will be responsible for conducting thorough assessments of clients' physical and emotional needs, developing and implementing individualized care plans, and collaborating with healthcare teams to ensure seamless patient care.
Responsibilities:
- Assess clients' needs and develop comprehensive care plans that address their unique requirements
- Collaborate with healthcare professionals to ensure coordinated patient care
- Provide education and support to clients and their families on disease management and self-care
- Manage caseloads effectively, prioritizing tasks and ensuring timely interventions
