Responsibilities : Position will include discharge planning and utilization reviewProvides social services to patients and their families, ensuring that medically related psychosocial needs are identified and addressedThis person assumes responsibility for discharge planning based upon individual patient needs and assists with utilization of resources for cost effective quality careContribute to positive work relationships in the Quality Services department, and with all coworkers in other departments.Focus on patient satisfaction and patient safety in the delivery of discharge planningAssist with utilization of resources for cost effective quality careAssist with the day to day utilization review activities for admission and continued stay reviews for all pay sources in the medical surgical unit, the ICU / PCU unit, a swingbed / skilled level of care, behavioral health unit, and OB unitAssist with monitoring medical records for documentation of medical necessity and discharge planningCollaborate with physician review activities on the nursing areas.
