Position Overview

This role offers an exciting opportunity to work collaboratively with interdisciplinary teams to develop and implement effective care plans for patients. As a key member of our team, you will provide expert guidance and support to patients, families, and healthcare professionals, ensuring seamless care transitions and collaboration.

Main Responsibilities

• Develop, implement, and update plans of care, incorporating evidence-based practice and quality improvement principles.
• Assist with patient assignments and determine patient acuity levels, ensuring timely and effective care delivery.
• Participate in case reviews and case conferences, sharing expertise and insights to optimize patient care.
• Communicate effectively with patients, families, and healthcare professionals, promoting seamless care transitions and collaboration.
• Stay up-to-date with relevant research, best practices, and industry developments, applying this knowledge to improve patient outcomes and care delivery.

Requirements and Qualifications

• Master's degree in nursing or related field
• Minimum 4 years of experience in a healthcare setting, preferably with a focus on patient advocacy and care coordination
• Advanced knowledge of patient assessment, care planning, and management principles
• Exceptional communication, interpersonal, and leadership skills
• Ability to think critically and make informed decisions in a dynamic environment
