About the Role

Responsibilities
- Collects data and conducts focused nursing assessments of patients' health status.
- Participates in developing and modifying patient-centered healthcare plans.
- Implements nursing interventions within patient-centered healthcare plans.
- Assists in evaluating responses to interventions.
- Maintains safe and effective nursing care rendered directly or indirectly.
- Advocates for patients' best interests.
- Communicates and collaborates with patients and healthcare teams.
- Provides health counseling and teaching.
- Delegates and assigns nursing interventions to implement plans of care.
Requirements
- Active license as a Licensed Practical Nurse (LPN) in the state of practice.
