Job Title: Licensed Practical Nurse in Long Term Care
About the Role:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Avera Marshall Morningside Heights Care Center. As an LPN, you will play a vital role in providing high-quality patient care and working collaboratively with our interdisciplinary team to promote the health and well-being of our residents.
Responsibilities:
- Provide professional nursing care, assessment, and evaluations to each resident within your scope of practice.
- Comply with all pharmaceutical policies and procedures, ensuring safe and accurate administration of medications/treatments according to physician orders.
- Conduct assessments on residents, make appropriate interventions, and prioritize resident care using the nursing process (assess, plan, implement, evaluate).
- Participate in the care planning process, ensuring consistent and age-appropriate implementation of care plans.
- Communicate and manage the delivery of resident care, ensuring quality care is provided.
- Facilitate conflict management while respecting diversity and promoting positive relationships.
- Guide neighborhood staff in meeting resident needs and implementing routines based on individual needs.
- Participate in physician rounds, communicate changes in resident condition, and utilize SBAR communication tool.
- Process and transcribe physician orders and implement according to facility policy.
- Identify staff performance issues and provide feedback through coaching, counseling, and conflict resolution.
- Enforce facility staffing policies and assist with data collection for quality functions.
- Seek educational opportunities, share knowledge with others, and assist with orientation of new staff.
- Maintain competency with operational equipment and investigate incidents/patient complaints.
- Administer medications according to facility policy and be competent in Pyxis medication administration system.
- Assist in discharge planning and education.
Requirements:
- Associate's degree from an accredited school/program or Registered Nurse (RN) - Board of Nursing active license in the state of practice upon hire.
- Preferred: Bachelor's degree from an accredited school/program and 1-3 years of experience in Long Term Care.
