Key Accountabilities
- Develop and implement individualized care plans, prioritizing resident needs and preferences.
- Conduct regular assessments, making appropriate interventions and adjustments as needed.
- Collaborate with interdisciplinary teams to coordinate care and services, ensuring seamless transitions and efficient resource allocation.
- Communicate effectively with residents, families, and staff, addressing concerns and providing updates on care plans and progress.
- Maintain accurate and detailed medical records, adhering to facility policies and regulatory standards.
Requirements and Qualifications
- Bachelor's degree in Nursing from an accredited institution or Associate's degree with relevant experience.
- Active RN license in the state of practice; certification in long-term care nursing is preferred.
- 1-3 years of experience in long-term care, preferably in a leadership or management role.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
Salary : $38 - $58
