SUMMARY: # The Patient Care Tech is responsible for providing quality patient care and assistance to the RN/LPN in caring for the personal needs of patients of all ages including, pediatric, adolescent, adult, and geriatric, under the direction and supervision of the RN/LPN. # EDUCATION and/or EXPERIENCE: Previous hospital experience, nursing home experience, nurse aid class or nursing student experience preferred. On the job training provided. Must be 16 years of age. # CERTIFICATES, LICENSES, and/or REGISTRATIONS:# Certified as a Nursing Assistant in the State of Iowa is preferred or needs to be obtained within 6 months of hire. Basic Life Support (BLS) required within 12 months of hire. Child/Adult Abuse certification required within 3 months of hire. # ESSENTIAL DUTIES AND RESPONSIBILITIES Knows the patient#s plan of care through attending bedside report, utilizing computer printout of shift report, and verbal communications with members of the care team. Anticipates needs of the patient according to age and condition; provides individualized care as directed by the healthcare team. Safeguards patient privacy in all cares provided. Answers call lights for all patients regardless of assignment.# Assists other team members as needed. Completes Hourly Rounds as assigned. Communicates effectively with all ages served and be able to communicate needs of the patient to RN/LPN team members. Aids with all ADLs to patients based on individualized patient needs.# This includes but is not limited to; dressing, bathing, repositioning patient, toileting/perineal cares, and oral cares with a goal to promote comfort and wellbeing.# Obtains vital signs, intake and output, and weight with accuracy.# Documents all cares provided from admission to discharge. Documents care appropriately in the electronic medical record. Assists with meal trays, nourishments and feeding of patients. Provides safe transportation of patients by wheelchair, cart/bed, or ambulation. Collects sputum, urine, and stool specimens.# Able to instruct the patient on proper technique. Handles, cleans, and properly cares for instruments and equipment, demonstrating good stewardship and infection prevention practices. Assists with admission/dismissal of patients. Prepares patient room after dismissal for next patient. Provides care within scope of practice as delegated by the RN/LPN. Other duties as assigned.
