Responsibilities
- Evaluates assigned patients, gathering data from various sources, including patient records, interviews, observation, and team members.
- Develops and implements individualized treatment programs for patients needing services, reviews plan of care with patient, and instructs patient, family, caregivers in home program or exercise as appropriate.
- Provides direct therapy services to patients using various functional settings and strategies, plans therapy within patient's individual physical, emotional, and intellectual capabilities as indicated and coordinated with rehabilitation, medical, and nursing care plans.
- Prepares documentation as required by profession and department, such as evaluation results, individualized treatment plans, progress notes, and other reports.
The Physical Therapist will work closely with interdisciplinary teams, including family and physician, to ensure comprehensive patient care and effective communication.
