We are seeking a new team member with leadership skills to develop a new care coordinator position. . Our clinic is open Monday - Friday, no holidays, 8 week Saturday morning rotation, 8-hour shifts. Consider applying today.
Job Summary
The care coordinator supports patient care navigation of clinically complex patients in the clinic setting, managing care for the patients of the clinic to promote effective education, self-management support, and timely healthcare delivery.
Education and / or Experience
Currently licensed as a Registered Nurse or LPN in South Dakota.
Previous experience in medical, surgical or pediatric nursing necessary.
Previous clinic experience preferred.
Leadership experience beneficial.
Licensure / Certification / Registration
