What You’ll Be Doing:
We are seeking a dedicated Service Coordinator to join our team and help us advance our mission. The ideal candidate will support and coordinate the delivery of services, aligning them with client goals within an interdisciplinary team. This role fosters communication between clients, treatment teams, external providers, and natural supports. Be a part of this journey with us! - Interdisciplinary Coordination: Collaborate extensively within an interdisciplinary team to align service delivery with client goals and objectives.
- Treatment Plan Management: Develop and monitor treatment plans, schedule appointments, document client interactions, identify programming challenges, and propose new services.
- Client Orientation: Conduct orientation for new clients and collaborate with the nurse to complete state assessments upon admission and as needed.
- Medical Care Coordination: Assist clients in establishing and maintaining medical care, including scheduling follow-up appointments and referrals for primary, vision, and dental care.
- Scheduling and Monitoring: Create and update weekly schedules for residents, organize and attend treatment team meetings, and ensure treatment plans are reviewed and revised according to company policies.
- Medication Support: Work with the nurse and staff to ensure clients receive necessary support with medications.
- Financial Assistance: Oversee resident pocket money, assist with budgeting, and help clients renew or apply for vital documents and insurance.
- Client Engagement: Assist clients with problem-solving, motivation, and engagement.
- Communication: Maintain direct communication with Program Manager, support staff, clinicians, skills specialists, and external care providers to organize client needs and goals.
- Family and Natural Support Coordination: Conduct weekly individual client meetings and family check-ins as needed.
- Progress Reporting: Provide monthly updates summarizing each resident’s progress.
- Resident Meetings: Facilitate monthly resident meetings to gather input and feedback for improving resident experience and treatment engagement.
- Therapeutic Milieu Maintenance: Ensure adherence to community expectations to maintain a therapeutic environment.
- On-Call Responsibilities: Share on-call duties with the Program Manager.
- Recreational and Social Activities: Plan and organize recreational activities, social events, and trips, including occasional overnight trips and weekend activities.
- Discharge Planning: Lead the discharge planning process, coordinating with clients, natural supports, and the TNE treatment team.

What You’ll Need:
- High School Diploma or equivalent
- Two years of experience working with individuals with disabilities in a service delivery field

- Associate degree from an accredited college or equivalent experience

Pay: $18.46 - $27.69 per hour
